I assume that there was some kind of handshake going on behind the scenes, but i was never really authenticated. Now, when I select Authentication > Windows Authentication I don't have the 'Providers' option in the right hand pane. Go to the site, click Authentication in the middle and then the middle frame has the authentication types.

Everything seemed set up correctly with the Application Pool Identity, but I continued to receive the error. I am running CA role on a Windows Server 2008R2. I suggest you look at PHP Manager information http://learn.iis.net/page.aspx/944/using-php-manager-for-iis-to-setup-and-configure-php/. You should be set to the default "Specific user: IUSR".
